Hello
I have installed TV Headend and have connected my new Pi3+ with its TV Hat directly to the aerial cable. Live tv struggles to appear irregardless of channel and when it does it either stutters as to be unwatchable or stops completely. I've tried different transmitters all to no effect.
When I plug the very aerial same cable into my Samsung Freeview TV I can watch fine.
Any help appreciated.

Are you sure it's a signal issue? My Pi 3 B+ struggled decoding and re-encoding even SD streams without some custom cut-down quality settings (and increased video RAM allocation if I remember rightly). What client are you using? Something that streams from the Pi using HTSP (Kodi with Tvheadend plugin or TvhClient on iOS) will eliminate the encoding being an issue. If you're trying to watch on the Raspbian desktop then I think you'll be out of luck.
