The Farnell parts are usually marked "Made in P.R.C" I have a Farnell part that exhibits the same issue as you describe, first write works, but then device doesn't appear as a mass storage device. I've not had any problems with the RS stocked parts that are made in the UK That's the only difference ...

+1 for under wattage PSU being used, seen that before causing flash corruption on a non-rpi device with GPRS modem.

Hi No I kind of gave up with that in the end! I've only managed to set initial levels via the main DT file (as per first post), which was good enough for what I wanted I did see one of the RPi engineers (James?) showing someone else a trick that allowed you to achieve something similar using overlay...
