I have been successfull with XMBC from OpenElec 3.1.6, by adding

"force_hdmi_open=1" to config.txt.

Strange things = the issues where stuff disappeared in the boot partition, leaving the Pi unbootable. And I still have this issue, every time (tried it 4 times) id create new image on 4GB Card I end up with a start.elf there in /boot and the md5sum is as expected, but after an init 6 the start.elf ...

Notes on previous post "My config"

card was a TRANSCEND 4GB class 6
USB Keyboard (standard cheap keyboard, but back lite)
no mouse was attached
Network fixedwire
power was adapter from RS (the one they say for a raspberry Pi)

Yes I want to do nearly the same thing! I hope to push the audio out over HDMI so it plays through my AV AMP, it has very high quailty DACs (24bit high sample rates etc) and can take MP3, DTS etc and decode them on the AMP! I just don't have a my PI yet to see if it will even put the audio out over ...
